Membership Application
Membership is available for a fee of $10 weekly, $30 monthly, $75 quarterly and $295 annually, for the first dog per household. A fee of $2 weekly, $5 monthly, &15 quarterly and $50 yearly will be added for each additional dog for the same household.

Access a Microsoft Word (2007) file of the application here. If you are using Internet Explorer, first ’save’ the document, then ‘file -> open’ the document from within Microsoft Word. (This file will save as a *.zip file on some computers. This procedure will still allow opening the file successfully.)
