The waiver states:
I understand that membership of my dog and dogs is not without risk to myself, member of my family or guests as well as my dog and dogs when using The Charlie Douglas Dog Park or any function at Bowser Minich Pet Crematory and Bowser Minich Funeral Home Inc.

I hereby waive and release The Charlie Douglas Dog Park its employees, owners and agents from any and all liability of any nature or kind, for injury or damage which my dog, myself, members of my family or guests may suffer, however caused, including specifically, but not without limitation, any injury or damage while using the dog park, the surrounding picnic and bench areas, the parking lot or while attending any training session or other function of The Charlie Douglas Dog Park.

In consideration of and as inducement to the acceptance of my application for membership, I hereby agree to indemnify and hold harmless The Charlie Douglas Dog Park, its employees, owners and agents from any and all claims, or claims by any member or guardians of my family or any other person on the grounds or the surrounding area thereto as a result of any action by an dog, including my own.

Addendum: For the protection of your dog or dogs and all the dogs who are members of The Charlie Douglas Dog Park, we require that all dogs registered for membership be current on their licenses, vaccinations and rabies.
